The Washington Administrative Code requires a long-term care assessment to be performed by a qualified assessor for every person prior to moving into an Adult Family Home. This is a necessary tool used by the Adult Family Home provider to develop an initial, annual and ongoing care plan promoting the resident’s optimum level of social, emotional and physical well-being. The assessment also helps the provider in determining the monthly cost for room, board and care.
The assessment includes a thorough review of:
- Social, physical and emotional strengths and needs
- Health history
- Medications
- Activity level
- Food preferences and special diet
- History of anxiety and depression
- Behaviors or symptoms that may cause concern or require special care
